You will be required to present the following documents, if applicable, at your interview: Driver’s license, passport for each citizenship claimed, certificate of citizenship (if applicable), proof of address or residency if different from application information (Utility bill, bank statement, address change info.) Birth Certificate: original or certified copy only, if needed for minor child or if you do not have a passport.
FOR MINOR CHILDREN: Birth certificate, passport (if applicable) AND permission/letter from both parents/guardians authorizing the child to obtain the travel document.
Please be prompt and timely, adhering to your appointment times. Check in at the office for your appointment and if there are persons in your party who DO NOT have appointments, please ask them to remain in the vehicle or out in the hallway. Only applicants with appointments may remain in the office during interviews. Thank you.
